Delivery Waiver

Deliveries are done curbside. If we are requested to dump any place else, a waiver must be signed.

This waiver is to release Asphalt Materials, SP Transport, or any other Trucks of any responsibility that may occur when take a truck past the curbside.

Delivering beyond the curb may result in additional charges to accommodate the additional truck time.

I have requested this procedure and have been advised of the risks for taking a truck past the curb. This damage could include but is not limited to, the curb and gutter, sidewalk, driveway, grass, sprinkler systems, personal property, etc. and I agree to indemnify and hold harmless Asphalt Materials and its affiliate for any and all damage that may be done.
